Many services are available at low or no cost, depending on eligibility, funding, and program guidelines.
Some programs require income verification or residency in specif ic townships.
Services are not guaranteed and may depend on available funding.
For eligibility questions, please review the program page first or call our office for guidance.
Birth and death certificates can be requested through our Vital Records office.
Before visiting, please review the requirements on our website to confirm:

- Processing time
Requests cannot always be cornpleted the same day, especially If additional verification IS required.

- Canton Main Office
Our primary location for most services - Astoria Satellite Office
Tuesday office hours for general questions
Clinic appointments on the 2nd and 4th Wednesdays - Cuba Satellite Office

Availability depends on staffing, program schedules, and demand.

- Proof of income
- Proof of residency
- Identification
For Cass and Putnam Township services. residents may qualify for one or more support programs.
Services are free, have no age restrictions, and require an application.
