Before You Call

Be ready to provide:

  • Household size
  • Basic income information
  • Who the program is for (pregnant woman, infant, or child)

This helps staff determine eligibility and next steps more quickly.

What to Bring

Bring the following to your appointment:

  • Identification (driver’s license, state ID, or passport)
  • Proof of income (30 days of pay stubs or last year’s taxes)
  • Insurance card, if applicable
  • Any required forms if provided in advance
